COMMON
AND SERIOUS DERMATOLOGICAL CONDITIONS : 22.2.2007
We promptly kicked off at 14:00 HRS ( at least I thought did )
The afternoon's proceedings were embroiled in a peculiar "to do.. or not to
do" sort of situation as far as the 'Hot Topic' was concerned. Everybody
agreed to keep things on simmer, until after!
There was a Premature and Prolonged ( if that's possible) discussion about
the 'Ethicality' of slipping someone a Sick note with a wink and agree to
take the rap till one's eye-balls turned green
( only coz we will do more of the same next week )
Finally we made our way Skin-deep.
The session was shared between the 3 of us - Amina, Sunaiyana and Rupen
( 2 from our group-2 were missed... sorry... I meant missing)
After coming to terms with the Lingo in true Dermatological fashion, we had
a slice of Sebhorreic Warts, Naevii, BCCs, Squamous Cell Carcinoma and the
ominous Melanoma. Having drummed up an appetite, we took a finger-licking
break to savour Alex's Chocolate something !!! and the-healthy-as-ever
crisps.
IT WAS BLOODY GOOD!
Amina and Sunaiyana had us lovingly interrogated and thrashed by more
creepy, flaky things ie. Eczema, Acne ( pustular & otherwise), psoriasis,
virally-bits, fungal broths and rashes of all colours to please the eye.

14.00 - we arrived
14.20 - we kicked off with a discussion prompted by Rupen about should he certify a bus driver fit to work after he had an apparent simple faint after a dodgy can of tinned pork. We agreed, that although there were no signs that would suggest epilepsy, Steve conveniently pointed out that its not our responsibility to decide whether he’s fit to drive but the DVLA (
http://www.dvla.gov.uk/medical.aspx )14.40 Swiftly on the Amina, and a rather interesting HOT TOPIC on off license prescribing. She went through what this meant and the rules surrounding. I’ve asked her to e mail me the presentation - I'll link it asap.
15.10 - Mary tried to rally the troops with a quick word about videos. I handed out some different marking sheets and tried (unsuccessfully I think) to explain that although videos are being dropped from the MRCGP, they are still a very useful teaching and assessment tool and there are different ways to mark them.
15.20 we abandoned the DVD as we couldn’t work out how to play the next chapter on the DVD.
15.25 Manuel’s wife provided some nice cake. Its definitely a nice theme to bake our own cake rather than buy from the faceless multinational corporations.
15.45 - we convened and split into three and started to watch the different videos. This was preceded by a video of a GP seeing a patient who had unexplained diarrhoea with fresh blood for 8 weeks. Diagnosis – IBS
15.50 - we all agreed that it was inappropriate not to refer this patient for further investigation and that IBS was a diagnosis of exclusion.
15.51 - my request for the consultation for relationship breakdown was denied.
15.55 - the groups split and we tried to work the TV.
16.00 - I dropped Steve's daughter's (kindly borrowed) TV but it still works - eventually.
16.20 - group three abandoned their TV and split to join the other groups
16.21 - my group watched Uzma and the very nice way that she handled an old lady with ?gout
16.55 - we reconvened (again) and reflected. we decided that it was a shame not to have seen more videos as they are useful and that if we get another chance that we should ‘carpe diem!’
We also discussed at the end
1 - We, as a group, really need to pull our fingers out and start on time as we’re wasting a fair bit of time milling around at the start.
2 - technology is a hindrance, not a help
3 - Gilly thought that there wasn’t as much of a buzz this week although Steve put this down to the time of year. 4 - Amina got her so desired session on orthopaedic problems in primary care.
5 - Chris encouraged the more quiet team members to get more involved
Amina to bring cakes next week - we’re hoping for home made Stollen and I'm doing the diary.
Next week Kidderminster, with Manjari doing the hot topic and group 1 presenting substance abuse.

14.00 - Most members arrived on or before 14.00hrs
14.15 – We kicked off, I can’t really recall any reason for the delay, Olu called for the meeting to commence, but I must say the drug representatives don’t make it any easier to start, with their lovely, assorted & eye catching "goodies".
14.17 – There was a brief pause as we all looked round if any one had a topic for discussion, eventually Muna gave a case presentation on Kawasaki disease in a 5yr girl (http://en.wikipedia.org/wiki/Kawasaki_disease).
We also had a brief discussion regarding the use of chaperons, some practices (GP) face the challenge of not having a chaperone as they occasionally have limited &appropriate manpower. 15.00 – Group 2 presentation on Substance Abuse by Manuel, Emma, Soumen
Manuel discussed on alcohol –Alcohol related problems, Recognizing a drinking problem, Managing a drinking problem, Managing alcohol dependence. He gave an in-depth discussion into the patient care and follow-up.
15.25 – Steve called for break- cakes, as the attentiveness of the group began to dwindle
15.45 – On returning from the break Sunil shared an alcohol related case – where the patient had finally accepted he had an alcohol problem after he had been suspended from work.

16.20 - she also divided the members into 6groups to facilitate a case study discussion on opioid misuse. We also discussed the use of methadone & detoxification.
